Bare Smiles Dental uses ICON - a resin infiltration treatment used for minimally invasive restoration of post-orthodontic white-spot lesions (WSLs) and certain congenital hypocalcified enamel lesions. WSLs are characterized by subsurface enamel porosities resulting from an imbalance between enamel demineralization and remineralization, usually caused by poor oral hygiene, plaque, bacteria, and acids. Over time, as remineralization occurs at the outer surface, calcium and other ions have reduced access to deeper enamel layers, eventually halting the lesion's progression. The white appearance of WSLs is due to light scattering within the demineralized enamel. On the other hand, congenital hypocalcified enamel lesions, also known as "hypo" spots or enamel bruising, are inherent enamel defects often triggered by trauma or infection during the development of primary teeth.
THE PROCEDURE
· one visit· no “shots” or numbing· no drilling· plain pumice is used to clean the teeth· lesions etched 3×2 minutes – this removes the outer remineralized layer of enamel that had previously been blocking calcium and other ions from entering the tooth· finishing bur on any surfaces that do not indicate an improved appearance· application of drying agent (ethanol)· application of the resin infiltrant· material soaks in for 3 minutes (allowing for capillary action)· remove excess material with cotton rolls, micro-brushes & floss· light cure the material· material added again for 1 minute, excess removed, and cured· finish with polishing discs or burs· temporary blanching of gums can occur in lesions close to the gumline* total process takes approximately 45 minutes to 1 hour *
BENEFITS
The benefits of Icon resin infiltration treatment include being much less invasive and more cost-effective compared to alternatives like fillings or veneers. It can also be done earlier than other cosmetic options such as veneers, which require waiting until growth is complete. Additionally, untreated hypo spots can appear worse after bleaching, but when treated with Icon, they whiten similarly to natural tooth enamel, providing a more aesthetically pleasing result.
LASTING POWER
In terms of effectiveness and durability, Icon resin infiltration is considered a long-term solution unlike whitening products. Research indicates stability for a minimum of 2 years, with many patients experiencing stable results for over 6 years. While the treatment may not guarantee complete disappearance of some spots, it typically offers a significant cosmetic enhancement in a single painless visit. In the worst-case scenario, if there is no change in appearance, the advantage is that no irreversible or invasive procedures have been performed on the tooth.
